  $2500/$5000 deductible for single/family. $0 co-pay after deductible. One doctor visit per year at no charge.

  Aetna HMO 10

  $10 HMO option. Primary office visit co-pay $10. Specialist co-pay $10. Hospital co-pay $0. Emergency Room co-pay $50. Prescription co-pay $15. Contraceptive co-pay $15.

  Aetna HMO 1

  Primary office visit co-pay $30. Specialist co-pay $50. Inpatient Hospital (per day/admission) co-pay $500 per day to $2500 max. per admit. Emergency Room co-pay $100. Prescription co-pay $20/$40/$70.
